  Whenitcomestohealth,thepooraredoublycursed.Notonlyaretheymorepronetodeadlyinfectiousdiseasesthantherich,buttheyhavefarlessaccesstothemeansofimprovement.Twentyyearsago,PaulFarmer,anAmericandoctorandanthropologist(人类学者),setouttodosomethingaboutthis.Amidthepoliticalturmoil(混乱)andpovertyofruralHaiti,hecreatedacommunitybasedhealthcaresystemcalledZanmiLasante,orPartnersinHealth.Itnotonlydeliversappropriate,affordablemedicaltreatmenttothousandsofpoorpeople,butgoesbeyondtheclinictoaddressthesocialcausesmakingthemsickandkeepingthemfromgettingbetter.

  AsDr.Farmerargues,improvingthehealthofthepoorisnotjustamedicalchallenge,butaquestionofhumanrights.Tacklingtheinequality,racism,sexismandotherformsof“structuralviolence”whichoppressthepoorisascriticalasextendingthedrugs.OrashisHaitipatientsputit,medicinewithoutfoodislikewashingone’shandsanddryingtheminthedirt.

  Unfortunately,Dr.Farmer’spowerfulmessageisoftenweakenedbyhisbook’sacademictone.Itdoes,however,screamoutinpassagesdescribingthehumanfaceof“structuralviolence”.ItisthesepersonalstoriesthatmakeDr.Farmer’sangeratsuch“stupiddeaths”socompelling.

  Thegooddoctor’smotivesandmethodsarebetterdescribedinMountainsBeyondMountains.ThisbiographybyTracyKiddertracesDr.Farmerfromhisunconventionalupbringingandunusualeducation,shuttling(来回穿梭于)betweentheshacksofcentralHaitiandthehallsofHarvardMedicalSchool,tohislaterworkaroundtheworld.Thoughwellwritten,Mr.Kidder’sbookalsomakesforuncomfortablereading.Theauthorisclearlyclosetohissubject,havingtraveledwithDr.FarmerfromthegreenpovertyofHaititothetubercularwhitenessofRussia.Tooclose,perhaps.Thebiographerseemstobeseekinghissubject’sapproval,ratherthantheotherwayround.Mr.Kidderwrites,ratherdisturbingly,abouthisfearofdisappointingDr.Farmer,hisownpainatwoundinghimwithacriticalremarkandhisreliefatthedoctor’sforgiveness.

  WhenMr.Kidder’shealthfalls,thisdependencebecomesallthemoreintense.Butratherthancompromisethebook’sequity(公正),thisintimacyservestohighlightDr.Farmer’sadmirable,yetultimatelyirritating,character.AsMr.Kidderobserves,“Farmerwasn’tputonearthtomakeanyonefeelcomfortable,exceptthoseluckyenoughtobehispatientsorthoseunluckyenoughtoneedhim.”

ThequestiontheenvironmentalcommunityandmanyinCongressareaskingiswhetherthiswouldhavebeennecessaryiftheriverhadbeenbettermanaged.Inpopulatedareas,someleveeswerebuiltsolelytoattractmoredevelopment,whileothersclosedofffloodplainsthatcouldhaveactedasanaturalsafetyvalve.

Meanwhile,overtheyears,theupperMississippiwatershedhaslostmillionsofacresofwetlandsthatcouldhaveservedasanaturalspongeforfloodwaters.
