本表为上海市外国留学生政府奖学金申请表（C类），在填写本申请表之前请详细阅读《上海外国语大学关于“上海市外国留学生政府奖学金申请试行办法”的实施细则》及《上海市外国留学生政府奖学金（C类）申请细则》，了解申请上海市外国留学生政府奖学金（C类）的资格条件及有关规定。
