[G]Anydoctorwhohaswrestledwithmultipleformsfromdifferentinsurers,orpatientswhohavetriedtounderstandtheirownparadeofstatements,knowthatsimplificationoughttosavemoney.Whenthehealthinsuranceindustrywasstillcooperatinginreformefforts,itstradegroupofferedtoprovidestandardizedformsforautomatedprocessing.Itestimatedthatstepwouldsavehundredsofbillionsofdollarsoverthenextdecade.Thebillswouldlockthatpledgeintolaw.

[I]Virtuallyallexpertsagreethatthefee-for-servicesystem—doctorsarerewardedforthequantityofcareratherthanitsqualityoreffectiveness—isaprimaryreasonthatthecostofcareissohigh.Mostagreethatthesolutionistopushdoctorstoacceptfixedpaymentstocareforaparticularillnessorforapatient’sneedsoverayear.Nooneknowshowtomakethathappenquickly.ThebillsinbothhouseswouldstartpilotprojectswithinMedicare.Theyincludesuchmeasuresasaccountablecareorganizationstotakechargeofapatient’sneedswithaneyeonbothcostandquality,andchronicdiseasemanagementtomakesuretheseriouslyill,whoareresponsibleforthebulkofallhealthcarecosts,aretreatedproperly.Forthemostpart,theseexperimentsrelyonincentivepaymentstogetdoctorstotrythem.

[J]Testinginnovationsdonogoodunlessthegoodexperimentsareidentifiedandexpandedandthebadonesaredropped.TheSenatebillwouldcreateanindependentcommissiontomonitorthepilotprogramsandrecommendchangesinMedicare’spaymentpoliciestourgeproviderstoadoptreformsthatwork.ThechangeswouldhavetobeapprovedorrejectedasawholebyCongress,makingithardfornarrow-interestlobbiestobendlawmakerstotheirwill.
