1、 批 准:Approved by IET批准:IET approval 陕西省机械施工公司扬巴项目部SMCC BYC Project Team2009年6月3日June 3rd, 2009一、作业概况 Operation Profile LDPE装置区15#变电站桩基工程施工区域C轴东侧7个桩位,桩位中心线B坐标2751.375,与施工区域内厂区污水管道空间位置距离较近,为确保地下管线的安全,拟在施工桩位处采用人工挖孔的方案明确管线具体位置。 具体作业方法:以桩位为中心,采取人工挖孔1200,120砖护壁,孔深为地面至水管底标高约3.2米,开挖涉及的地基土层主要由杂填土,素填土,粉质黏土组成。

2、In Substation 15 (LDPE), east side axe C of the piling construction area, there are seven piling positions (coordinate of piling central line B 2751.375), which is close to drainage pipelines within the construction area. In order to guarantee the security of the underground services, manual excavat

3、ion of piling holes will be adopted to clarify the specific location of underground services. The specific execution method is as follows: piling holes will be excavated manually (1200mm), and 120 brick wall will be used. Elevation of hole is approximately 3.2 m from ground to bottom of pipeline. Th

4、e soil of foundation in the excavation area is composed of miscellaneous fill, plain fill and silty clay. 二、作业特点及风险分析 Operation Features and Risk Analysis该作业比较显著的作业特点是劳动强度大,挖孔速度慢,作业环境较差,安全隐患较多。根据我单位的施工经验,经过风险分析,判断可能造成的事故类型如下:The most prominent feature of the operation is labor intensive. The hot dig

5、ging process is slow. The operation environment is relatively bad with potential safety hazards. Based on construction experience and risk analysis, the possible causes of accidents are presented as follows:(1)人、坠物落掉入桩孔造成事故;Staff and objects fall into the piling holes.(2)孔内有害气体含量超标造成事故;Hazardous gas

6、 in the hole is above the standard.(3)塌孔造成事故。Piling holes collapse.(4)触电事故。Electric shock hazard三、作业前的安全准备 Safety Preparation before Operation (1)组织相关人员学习相关的法律、法规,了解掌握IET及本公司的规章制度,提高安全责任意识。Organize relevant staff to learn laws and regulation to have a good command of the IET and SMCC HSE rules and r

7、egulation.(2)组织施工技术人员熟悉施工工艺和地质勘察报告,研究制定作业控制程序,明确卡控的重点及责任人,并将此内容详细交底给作业人员。具体施工工艺及施工图详见执行方案。Get construction technical staff to be familiar with construction process and geotechnical survey report. Operation control procedures shall be worked out. Persons who are responsible for key process shall be i

8、dentified. The details shall be presented to working staff. For the detailed construction process and drawing, please refer to construction execution plan.(3)作业人员必须接受项目部的安全交底和技术交底,必须参加IET组织的安全入场教育、受限空间作业培训,并学习需要的操作规程。上述四项落实后方可开始作业。All the working staff shall receive HSE and technical presentation, I

9、ET HSE induction, restricted work training and all the necessary training for the operation procedures. Only all the trainings have been finished, operation is allowed.(4)机具设备进场后必须经过项目部检查,并经 IET-HSE管理人员验收同意后方可投入使用。Equipments can only be used after checked by project team and accepted by IET HSE staf

10、f.(5)发放齐全的PPE给作业人员,同时确认其是否掌握和了解上述安全要求、作业程序、安全技能等。PPE shall be handed to working staff. Make sure all of the staffs are informed of the abovementioned safety requirement, working procedure and safety skills.四、作业过程的安全分析及应对措施Job Safety Analysis of Operation and Corresponding Measures 1、重点控制的安全措施 Key co

11、ntrolled HSE measures 在进行人工挖孔桩施工时,首先应做好孔口围护措施。根据建筑桩基技术规范(JGJ9408)规定,井孔周边必须设置安全防护围栏,高度不低于1.2m,正在开挖的桩孔停止作业或已挖好的成孔,必须设置牢固的盖孔板,非工作人员禁止入内。孔内作业时,孔口上面必须有人监护。挖出的土方应及时运离孔口,不得堆放在孔口四周1m范围内。孔内作业人员必须头戴安全帽、身系安全带。利用吊桶/吊兰运土时,必须采取相应的防范措施,以防落物伤人。施工中应随时检查孔壁情况。Fence measures shall be taken to protect piling holes durin

12、g the manual excavation process. Based on “construction piling technical standard” (JGJ9408), security fence shall be set up surrounding the piling holes. The height shall be no less than 1.2m. Cover plates shall be used to protect excavating or excavated holes. Non-working staff is not allowed to e

13、nter the site. If working within the holes is needed, guards are needed in the area surrounding the holes. Soils excavated shall be removed away from the holes, which cannot be stored within 1m surrounding the piling holes. Staff working under the holes shall wear helmets and harnesses. Lifting buck

14、et or basket shall be used to remove soil. Prevention measures are needed to protect subject or staff from falling. During the construction process, soundness and holing wall condition of vertical be checked frequently.2、人落入孔内的预防措施 Measures to protect staff from falling into the holes为了防止作业人员在上下时坠落,

15、不得用人工拉绳子运送人员或脚踩凸缘上下桩孔,应使用安全爬梯上下,孔内必须设置应急爬梯或设置尼龙绳,并随挖孔深度增加延长至工作面,供人员应急使用。井孔内必须设置应急时使用的安全绳和爬梯。井内人员不得乘坐吊桶/吊兰或脚踩护壁上下井孔。In order to protect working staff from falling into holes, it is not allowed to use lines to transport working staff manually or enter the holes by stepping the sides of pile holes. Saf

16、ety ladders shall be used for ups and downs. Emergency ladder or nylon lines shall be used in the holes, which shall extent to the working surface as the depth of the pile deepens. Security line and ladder shall be set in the holes for the sake of emergency. Staff working under the holes is not allo

17、wed to use basket or bucket to go down or up the holes.3、坠物落入孔内的预防措施 Measures to protect objects from falling into the holes作业人员为图方便,常常将挖出的土在孔边堆得很高,造成严重的安全隐患。因此,在对工人进行管理的同时,应安排专人把挖出的土石方及时运离孔口,且不得堆放在孔四周1米范围内。开挖时采取分段开挖、分段护壁,每段挖深为1.0m,护壁随着孔深增加而往作业面下引。孔深挖至超过挖孔人身高时,在桩孔口或孔内装设半圆防护板,吊渣桶上下时,孔下作业人员应避于护板下。Exca

18、vated soils are usually stored high surrounding the piling holes which will cause potential hazard. Thus it is necessary to remove the excavated soils timely away from the piling holes. Soils excavated shall be removed away from the holes, which cannot be stored within 1m surrounding the piling hole

19、s. Excavation shall be executed section by section, and so does protection walls. Each section of excavation is 1.0m. Protection walls shall deepen along with the depth of the piling holes. When piling holes are of a persons height, semi-circle protection plate shall be used either at or within the

20、piling hole. Staff working under holes shall be stayed under the plate when the lifting basket comes up and down.4、垂直运输工具失灵的预防措施Measures to protect vertical transportation vehicles from dis-functioning在开工前应该检查垂直运输工具的是否完好,防止施工过程中吊索断裂导致的物体打击。使用的吊桶/吊兰应安全可靠,上班前、下班后均应有专人严格检查。Check whether vertical transp

21、ortation vehicle are in a good condition or not. Measures shall be taken to protect lifting basket from breaking and objects from falling. lifting bucket and lifting basket shall be inspected before and after work. Make sure machine has oiled every day.5、孔内有害气体含量超标造成事故的预防措施Measures to protect hazard

22、ous gas from exceeding the standard地下特殊地层中往往含有CO、SO2、HS2或其它有毒气体,依据相关规范和经验,当人工挖孔桩深度超过5m时,就必须进行有毒气体的检测。该作业挖深3.2米,基本可以不采取相关措施,且经过与BYC沟通,确认开挖范围无其他化工类排放管道,基本确定开挖不会产生有毒气体。但仍然必须注意:在施工过程中防止出现施工人员缺氧窒息事故发生,随时提醒并要求井下作业人员依据自身感觉判断。Hazard gas such as CO, SO2 and HS2 can be found in the stratum. According to relev

23、ant standard and former experience, when the piling is over 5m deep, hazard gas test shall be conducted. The hole is 3.2 m deep, and thus relevant measures are not required. After the communication with BYC, it is confirmed that no other chemical pipelines are found in the excavation area. Thus no h

24、azard gas can be generated in the excavation. However attention shall be aroused to protect staff members from suffocating. Remind staff working under the holes shall make judgment based on personal experience.作业前通过IET施工负责人,与扬巴LDPE装置区取得联系,委托其安排专业有毒气体检测工为我单位提供井下气体检测服务。作业前准备好一台鼓风机,在挖深超过井下人员身高后,采用鼓风机进行

25、送风,确保通风良好。在井底放置鸽子一只,井下作业人员应及时观察鸽子的安全状态。Before operation, SMCC shall contact BYC LDPE area through IET team, authorizing LDPE to conduct underground gas testing by hazard gas testing staff. Before operation, air blower machine shall be prepared. After the depth is over a persons height, air blower ma

26、chine shall begin to work to keep air circulation in good condition.如果一旦发生人员中毒、窒息等事故,必须在现场按应急措施规范要求实施抢救,根据情况及时送医院进一步抢救治疗,并依据安全计划中第15章事故报告程序执行,以便采取相应措施。Any incidents such as poison or suffocating happening, emergency measures shall be taken for first aid. “Accident report procedure” in Chapter 15 of

27、“HSE Plan” shall be adopted.6、防触电措施Measures to protect from electric shock本作业初步分析在开挖中应不会出现地下水抽排工作,但如果出现地下水,需要使用污水泵时,要求抽完水后才允许挖孔,并且采用动作电流为15mA的漏电保护器。No underground drainage will occur in the preliminary phase of the operation. If underground water occurs, drainage pump will be used. Only excavation i

28、s allowed after the drainage. 15mA leakage protection device will be used.如果地下涌水量较大,工人挖桩时往往会一边抽水一边开挖。因为如果抽水时工人在桩外等侯,待抽干水后再进入内挖桩,此时桩内的水又冒了上来,根本无法往下挖,这样施工就很容易触电。因此,为保证安全施工,解决挖土与抽水的矛盾,可用深水泵代替潜水泵抽水,因为深水泵抽水时是将抽水管伸到桩内抽水,管不带电,这样就可以边抽水边挖土。If the underground water is in large amount, workers shall pump the w

29、ater at the same time of excavation. The reason for this is as follows: if pumping and excavation are conducted separately, electric shock can happen to working staff. Thus in order to guarantee the safety of the construction, deep water pump can be used, because deep water pump can extent into the

30、inner of the piles and the pump is free from electricity, which will make pumping and excavation happen at the same time.施工现场的一切电源、电路的安装和拆除必须由持证电工操作。用电设备必须严格接地或接零保护且安装漏电保护器,各桩孔用电必须分闸,严禁一闸多用。孔上电缆必须架空,严禁拖地埋压土中,孔内电缆、电线必须采用护套等有防磨损、防潮、防断等保护措施。孔内照明应采用安全矿灯或36V以下的安全灯。孔中操作工应手戴工作手套,脚穿绝缘胶鞋。Assembling and disas

31、sembling of power and circuit at the construction site shall be operated with qualified electricians. Electrical devices shall be earthed or zero protected with leakage protection devices. Separate brakes shall be installed for each piling holes. One brake for more than one hole is forbidden. Cable

32、shall be kept away from ground and buried in the earth. Cable and wires shall have protection casing against damage, moisture and breakage. Lighting in the holes shall use safety mining lamps or safety lamps under 36V. Operators in the holes shall wear gloves and insulation shoes.7、塌孔的预防措施Measures to protect the holes from collapsing塌孔是人工挖孔桩施工中对工人的生命安全最大的威胁之一,其产生的原因及应
