因私出国申请书

　　因私出国申请书

　　普通护照是指发给一般公民使用的护照。

　　在我国,普通护照又分为两种,一种是因公普通护照,一般发给国家派出的研究生、留学生、访问学者和工程技术人员等,这种护照由外交部或地方外办颁发。

另一种是因私普通护照,由国家公安部或公安部授权的各地公安机关颁发给因私事前往外国或旅游居住在国外的本国侨民使用的护照。

　　1、党政机关副处级以下因公临时出国人员。

　　2、除外商独资企业、私人企业以外的各企业、事业单位从事经贸、科技等活动的人员。

　　3、参加各种非政府间国际学术组织和国外学术团体举办的学术会议的人员，及参加中外学术团体、教育机构间互访的人员，单位派往国外讲学、任教和工作的人员。

　　4、民间组织的出国人员。

　　5、新闻机构出国人员。

　　6、出国演出或举办展览的人员。

　　7、出国参加比赛或训练人员。

　　8、国内远洋公司海员。

　　9、从事对外经济援助的人员。

　　10、常驻国外民间机构的人员。

　　11、到国外进修的中科院和工程院的院士、高等院校的校长、副校长及相当这一级别的人员。

　　12、为执行科技合作协议，并持有《执行科技合作协议出国合作研究、进修人员登记表》的出国进修人员。

　　13、颁发护照机关认为需持因公普通护照的人员。

　　护照有一定的有效期限，过期即为无效护照，丧失法律证明效力。

我国因私普通护照有效期一般为5年，可以延期两次，每次不超过5年。

从20XX年开始一般都是10年了。

　　办理护照的方法？

　　第一部分申请办理护照的程序

　　护照是公民在国际间通行所使用的身份证和国籍证明，也是一国政府为其提供外交保护的重要依据。

为此，北京市居民出国旅游，需要申请办理护照。

具体程序是：

　　1、携带本人身份证或者户口簿到户口所在地派出所、公安分县局出入境管理部门、北京市公安局出入境管理处或者参团

　　旅行社领取《中国公民因私出国申请审批表》

　　2、填写申请表。

　　3、提交申请。

提交申请时必须携带下列材料：

　　a、本人身份证或者户口簿原件和户口簿首页、本人资料页、变更页及身份证的复印件。

　　b、填写完整的申请表原件。

　　c、符合要求的彩色照片一张。

　　4、领取护照。

北京市公安局出入境管理处受理申请后，审批、制作和签发护照的时间是14个工作日。

领取护照时，须携

　　带本人身份证或者户口簿、领取护照《回执》和200元工本费。

凡在《回执》上标明取证日期3个月后没有领取证件的

　　北京市公安局出入境管理处将予以销毁。

　　第二部分填写申请表

　　填写《中国公民因私出国申请审批表》一定要认真细致，逐项如实填写。

在填写时，必须使用黑色或者蓝黑墨水，字迹清楚整洁，不准涂改。

具体项目的填写要求是：

　　第一页：

　　“申请人姓名”,填写本人姓名；

　　“受理日期”等项目无需填写；

　　第二页：

　　“姓名”，填写本人现用名，须使用国家标准简化字，与户口簿和居民身份证一致；

　　“拼音姓名”，须按普通话拼写；

　　“出生日期”，与户口簿和居民身份证一致；

　　“民族”，与户口簿和居民身份证一致；

　　“公民身份证号”，逐格填写；

　　“出生地”，填写省、自治区、直辖市；

　　“婚姻状况”，按实际情况填写；

　　“户口所在地”，填写申请人户口所在地的详细地址；

　　“户口所属派出所”，按户口簿上派出所的印章名称填写；

　　“政治面貌”，可以填写“中共党员”、“共青团员”、“群众”或者民主党派的名称；

　　“文化程度”，填写国家主管教育部门承认的最高学历；

　　“职业、职务、职称”，按申请人现状况和国家承认的“职称”填写；

　　“工作单位”，填写全称，申请人人事档案与工作单位不一致的，填写现工作单位，在备注栏内注明档案存放地;退休人员人事档案

　　存放在原单位，填写原单位；

　　“联系电话”，填写可以直接联系的电话；

　　“家庭住址”，填写现住址；

　　“邮政编码和联系电话”，按实际情况填写；

　　“前往国家或者地区”，填写此次拟前往的第一个国家或者地区；

　　“属第次申请因私出国”，指在公安机关申请出国的次数，未被批准的也合并计算，并在备注栏内说明未被批准

　　的情况；

　　“出境事由”，选择“旅游”一项；

　　“本人简历”，从初中开始，起止日期要准确、衔接；

　　第三页：

　　“境外邀请人基本情况及申请人的关系”，填写“自费出国旅游”；

　　“国内外家庭主要成员”，按实际情况填写，境外人员的单位和地址用中文填写；

　　“备注”，填写需要说明的情况；

　　第四页：

　　“单位或者派出所意见”，填写是否同意申请人出国。

意见内容包括：

申请表所填内容是否属实、申请人是否具有法定不准

　　出境的五种情形和是否同意申请人出国等。

负责出具意见的人员必须签名、加盖公章、注明准确的联系电话和日期。

单位或

　　者派出所意见自签发之日起6个月内有效。

　　法定不准出境的五种情形：

　　1、刑事案件的被告人和公安机关或者人民检察院或者人民法院认定的犯罪嫌疑人；

　　2、人民法院通知有未了结民事案件不能离境的；

　　3、被判处刑罚正在服刑的；

　　4、正在被劳动教养的；

　　5、国务院有关主管机关认为出境后将对国家安全造成危害或者对国家利益造成重大损失的。

　　关于单位或者派出所的意见，需要说明的是：

　　国家公职人员，由其组织人事部门按干部管理权限出具意见。

　　国有企业和国有控股企业和事业单位员工，由所在企、事业法人代表或者其授权的单位人事、保卫部门签署意见，法人代表

　　本人出境，由相应的上级人事管理部门出具意见。

　　14周岁以上的在校学生，由所在学校出具意见。

未满14周岁的，由其父母或者监护人出具意见，免交单位或者

　　派出所的意见。

监护人是祖父母、外祖父母或者近亲属的，须提交能证明亲属关系的证明，如派出所或者监护人单位出具的

　　亲属关系证明和能证明亲属关系的户口簿等。

　　上述类人员以外的申请人，档案存放在全国人才交流中心、北京市职业介绍服务中心的，可由“中心”

　　出具意见；档案存放在北京市区人才交流中心的，可分别由“中心”和区人事局或者劳动局出具意见。
