Restrictedspaceentryprocedure

1.programpurposes

1.1thisprocedureisusedtoguidethesafeaccessofallconfinedspaceoperators.

1.2providesafeworkingconditionstothoseinvolvedinconfinedspaceoperations.

2.scopeofapplication

ThisprocedureappliestoallDujiangyanLafargeIndiaCementsLimitedlimitedspaceoperations,employees,contractorsandthirdpartymustabidebythisprocedure.

3.definitions

Confinedspaceentrypermitmustbeauthorizedforworkerstoenterthisarea:

designednotforcontinuoususe;sizeandshapetoallowworkerstoenterthebody;importduetolimitedopeningbecomesdifficult;andoneormoreofthefollowingcharacteristics:

（1）toxicgasesthatexistormayendangerentry.

（2）becauseofstructuralreasons,naturalventilationisinsufficientornearthesourceofpollution,whichmaycontaindangerousconcentrationsoftoxicgases,flammablegasesorvapors,orinsufficientorexcessiveoxygen.

（3）mayengulfentrants.

（4）becauseoftheinternalstructuremaybeenteringtheinjured,suchasinclinedwall,groundtiltetc..

（5）thereareotherserioussafetyandhealthrisks.

Thereisadangerinthe15items,andtheopenspacewiththeheadaccessibleisregardedasconfinedspace.Evenifthemouthdoesnotallowthebodytoenter,soshouldbetreated.

Iftheperson'sbodybreakstheplaneoftheopeningoftheconfinedspaceandtheopeningsizeislargeenoughfortheheadtoenter,eveniftheworkerdoesnotputhisheadintotheopening,arestrictedspaceshouldberequiredtoenterthepermit.

Confinedspacesincludeboilers,stoves,tanks,drains,puddles,largepipes,pipes,filterbags,coolingtowers,andsilos.

4.responsibilities

4.1equipmentoperators,DepartmentHeads/managers,maintenancesupervisors,entrypersonnel,on-sitesupervisors,safetypersonnel,lifeguardsandsupervisorsshallcomplywiththeseregulationsandperformthedutiesintheprogram.

4.2theplantmanagerandgeneralmanagershallensurethattheproceduresareproperlyimplemented.

5.program

5.1thesitesupervisororsupervisorshallensurethattheworkareaisreadyasfollows:

1.ensurethat"dangerousrequirementspermitrestrictedspace,donotenter"signswhenplacedateachentrypoint.

2.unlessthereisanincreaseintheoverallriskofentry,alifelineshallbeattachedtotherestrictedspace.Ifnoropeisattached,theequipmentsupervisor,maintenancesupervisorandsafetyofficershallbeapproved.

3.iftheconfinedspaceneedsmorethan1.8metersofverticallifting,mechanicalrescueequipmentshouldbeprovidedandplacedintherescuearea.

4.removehazardoussubstancesbymeansofemission,purification,etc..

5.ensureproperventilationandsafetydevicesareinplaceandingoodcondition.

6.alltoolsaresuitableforconfinedspaces.

7.preparepersonalprotectiveequipmentandcarepersonnelcontactdevices.Ifbreathingapparatusisneeded,providebreathingapparatusforatleast5minutestoeachentry.

Thedepartmenthead/managershallarrangethequalifiedpersoninchargeofthesite.

8.ensurethatotherworkpermitsthatmayaffectrestrictedspaceaccesstopersonnelarenotissued.

9.rescueplansshouldbeprovidedandrescueteamsshouldbetrainedinthistypeofrescue.Qualifiedrescuepersonnelmustreviewtheplanorprepareanewplanasneeded.

5.2thepersonresponsiblefortheworkshallbeinaccordancewiththeproceduresforisolatingtheenergyfromtheplant,

Allenergysourcesarelockedandanenergyisolationtableshallbepreparedtoshowallpotentialenergysourcesthatmayaffectthework.

5.3workpersonnelconductpreliminarytestsofoxygen,explosionlimits,andtoxicsubstancesinsequence.Ifcontaminantsarepresent,retestthematthefrequencyspecifiedbythesecuritydepartment.Updatetheinspectionrecordofallcopiesofthelicenseafterretest.

5.4operators,maintenance/supervisors,managers,operators,andallotherpersonnelenteringtherestrictedareashallreviewandsignoperationalriskanalysistoidentifyrisksinthework.

5.5sitesupervisor,DepartmentManager,safetyofficerandGeneralManagerapproveandsignlimitedspaceaccess.Thevalidityofthepermitshallnotexceed8hours,andthespecialcasemaybeextendedto12hours.Fortheconfinedspaceworkdaily,thegeneralmanagercannotatthesceneofapproval,suchasmaintenance,repair,grindingcementrawmillinpreheatercyclonecleaningskinningoperations;foroccasionalconfinedspaceoperations,thegeneralmanagershallberecognizedinthefieldsignature,suchasclearlibrarywork.

5.7restrictedspaceentrypersonnelpostedonthejobsiteaccesstorelevantworkpermits.

5.8allentrantsmustwearpersonalprotectiveequipmentbeforeentering.Iftheconfinedspaceinmemoryinhigh-altitudeoperations,eachentrypersonnelmustwearfullbodysafetybelthook,andhunginasolidanchorpoint.

5.9rescueropesshouldbepreparedforrescueneeds.

5.10inconfinedspaces,thejobguardianshallalwaysmaintainaccuratecalculationofthenumberofpersonsinconfinedspacesandensurethatthenumberofpersonsintheconfinedspacewillnotexceedthenumberavailableintheconfinedspace.Theguardiankeepsasignin/checkoutrecordtoensurethateachentry'sfullname,dateandtimeofentryandexitarerecorded.

5.11theheadofoperationsmonitorstheworkareathroughouttheworktoensurethesafetyofthework.

5.12whenapersonisconfinedinaconfinedspace,heorsheshouldalwaysremainattheentranceandmaintaineffectivecontactwiththeentrant.Ifpossible,observetheentrant.Evacuationshouldbeorderedincaseoffire,gasleakage,emergencywarningorotherdangerousincidents.

5.13forthelimitedspacetherearetoxicorharmfulgases,monitoringpersonnelshouldcontinuouslymonitorthegasmonitoring.Ifreadingchangesormonitoralarm,immediatelynotifytheleftintotheconfinedspace.

5.14,whennewpersonneljointhework,shouldnotifythesceneofthepersonincharge.Newentrantsshouldensurethathis/hernameisaddedtothecopyofthepermitandtothelimitedspaceworkriskanalysissheet.Andclearaccesstime,departuretime.

Ifaworkerisremovedfromhispost,he/sheshallensurethathis/hernameisremovedfromthepermitcopyandnotifiedtothesiteoperatorwhohasbeenremovedfromhispost.

5.16incaseofemergency,theguardianshouldfirstrequestassistancebywalkietalkieontheway,andrequestthatifnecessaryaftertherescue,hewilltrytorescuetheentrantwithoutentry.

5.17retestoxygen,explosionlimits,andtoxicsubstancesafterthealarmhasbeenlifted.Reverifythesafetyrequirements,resignthelicense,includingthedateandtimeofresigning.

5.18whenenteringthework,theentryshallcomplywiththefollowingprocedures:

Enterthestaffshouldbeonthepermitformarkingisnotcomplete,fillinthedateandtime,thesummaryoftheworkschedule,workpermittothedepartmentorresponsibleperson（theresponsiblepersonshallensurethatsupervisorsreceivelicense）.

Iftheworkhasbeencompleted,enterthelicensetocompletepersonnelwillmarkandfillinthedateandtime,inthelastpartofthepermittosigntoconfirmthefinaloutofthestaffandentertheworkdone,returnauthorizationformtothesupervisororthepersoninchargeofsite（siteresponsiblepersonshallensurethatsupervisorsreceivelicense）.

6..Manholemanagementsystem

6.1round:

fortheconvenienceofoperatingpersonnel,inspection,measurement,cleaning,inmechanicalequipment,pipes,containers,warehousesandotheropenhole.

6.2DepartmentManager（supervisor）isresponsibleforthesafetymanagementofthemanholedepartment,isresponsibleforensuringthenormalworkofmanholeinasafestate.

6.3employeesneedtoopenthemanhole,wemustdotheworkofriskanalysis,andapprovedbydepartmentsupervisorormanager

6.4approved,openthemanhole.

6.5afterthetaskhasbeencompleted,thepersonmustberestored

6.6ifyouneedtoentertheconfinedspaceoperation,youmustalsoworkintheenclosedspaceworkpermit.

7.confinedspacerescueteam

7.1forthefollowingdepartmentsinthefactory,arestrictedspacerescueteammustbesetuptocarryoutrescuetasksincaseofemergency.

7.2therescueteamisresponsiblefortherescuetaskinthelimitedspaceemergencyoftheDepartment,andcanparticipateinthelimitedspacerescuetaskinspecialcircumstances.

7.3therescueteamshallensuretraining,befamiliarwiththeuseofgasdetectors,rescuethreetripods,basketstretchers,positivepressurebreathingapparatus,andmasternecessaryfirst-aidknowledge.

8.managementofemergencyequipment

8.1,thefactorylimitedspaceemergencyrescueequipmentare:

positivepressureairbreathingapparatus4sets,threefootRescueKit2sets,basketstretcher1sets,medicalstretcher2sets,threeinonegasdetector4sets.

8.2thepositivepressureairbreathingapparatusiskeptinthecentralcontrolroom,andthecentralcontrolroomoperatorisresponsibleforcleaningandusingrecords.

8.3,threestandrescuekit,themanufacturingdepartmentisresponsibleforthemanagementof1sets,themechanicalexecutiondepartmentisresponsibleformanaging1sets,andisresponsibleforrecordingtheuse.
