（c）IunderstandthattheinformationcontainedinthisapplicationwillbedisclosedbytheSirDavidTrenchFundCommitteetorelevantgovernmentdepartments/organisationsforthepurposesofassessment,monitoringandevaluationofapplications.Iftheapplicationisapproved,therelevantinformationincludingnameoftheapplicant,thedetailsoftheprojectsappliedandtheapprovedamountwillbepublishedonthewebsiteofHomeAffairsBureauandtheSirDavidTrenchFundforRecreationTrustees’Reportforpublicaccess.

ThepersonaldataprovidedintheapplicationformwillbeusedbytheSirDavidTrenchFundCommittee（“theCommittee”）anditsSecretariatforthepurposeofassessingapplicationstotheMainFundoftheSirDavidTrenchFundforRecreation.Forsuccessfulapplications,suchdatawillalsobeusedforprojectmonitoring,promotion,publicityanddisseminationpurposesasappropriate.
Theprovisionofpersonaldatainthisapplicationformisvoluntary.Thelackofcertaininformationmayaffecttheassessmentoftheapplication.
這份申請表內填報的個人資料，將會供戴麟趾爵士康樂基金委員會（“委員會”）及其秘書處，用作審批戴麟趾爵士康樂基金（主要基金）申請之用。
此外，成功申請的計劃書內的個人資料，亦將會被用作監察、推廣及推介計劃之用。
在這份申請表填報個人資料純屬自願性質。
如未能提供某些資料，可能會影響申請書的評審。

PersonaldataprovidedinthisapplicationmaybedisclosedbytheCommitteeanditsSecretariat,ifnecessary,tootherGovernmentbureaux,commissions,departmentsandotherpeopleconcernedforthepurposementionedabove.
為了審批戴麟趾爵士康樂基金（主要基金）的申請，委員會可能會把這份表格所載的個人資料，向其他決策局、委員會、政府部門及其他有關人士披露。
AccesstoPersonalData查閱個人資料
Applicantshavetherighttoaccessandcorrectthepersonaldataprovidedinaccordancewithsections18and22andPrinciple6ofSchedule1ofthePersonalData（Privacy）Ordinance.Theirrightofaccessincludestherighttoobtainacopyoftheirpersonaldataprovidedintheapplicationform.
