Background
Bipolardisordersareamajorpublichealthconcern.Effortstoprovideoptimalcarebygeneralpractitionersandpsychiatristsareunderminedbythecomplexityofthedisorderanddifficultiesinapplyingclinicalpracticeguidelinesandnewresearchfindingstothespectrumofcasesseenindaytodaypractice.
Method
Anationalnetworkofbipolarexpertcentreswasestablished.Eachcentrehasestablishedstronglinkstolocalhealthservicesandprovidessupporttocliniciansindeliveringpersonalizedcareplansderivedfromsystematiccaseassessmentsundertakenatthecentre.
Results
Acommonsetofdiagnosticandclinicalassessmenttoolshasbeenadoptedateightcentres.Evaluationsareundertakenbytrainedassessorsandcross-centrereliabilityismonitored.Awebapplication,e-bipolar©
isusedtorecorddatainacommoncomputerizedmedicalfile.Anonymizeddataisenteredintoasharednationaldatabaseforuseinmulti-centreauditandresearch.
Conclusions
Insteadofofferingtreatmentadvicebasedonclinicalpracticeguidelinesrecommendationsforselectedsub-populationsofpatients(a‘top-down’approach),theFrenchbipolarnetworkofferssystematic,comprehensive,longitudinal,andmulti-dimensionalassessmentsofcasesrepresentativeofgeneralbipolarpopulations.This‘bottom-up’strategymayofferamoreefficientandeffectivewaytotransferknowledgeandshareexpertiseasthereferrercanappreciatetherationaleunderpinningsuggestedtreatmentprotocolsandmorereadilyapplysuchprinciplesandapproachestoothercases.Thenetworkalsobuildsaninfrastructureforclinicalcohortandcomparative-effectivenessresearchonmorerepresentativepatientpopulations.
