1SCOPE
ThisprocedureappliestoContractor’semployees,Subcontractor’semployeesandvisitorsenteringtheLLD/HDPE–PP–LDPEwithafallexposureof1.8meterorgrater.
2PURPOSE
Toestablishminimumrequirementsandguidelinestoprovidemaximumprevention/protectionagainstfallsfromelevationandaminimumstandardoftrainingnecessarytoensurepersonnelunderstandingandcompliancewiththeprogram.
Thepurposeistoachieve100%fallprevention/protectionforallpersonnelexposedtopotentialfalls.
3DEFINITIONS/ACRONIMS
ANCHORAGEAsecurepointofattachmentforlifelines,lanyards,ordecelerationdevices.
BUCKLEAnydeviceforholdingthefullbodyharnessclosedaroundtheemployeesbody.
CONNECTORAdevicethatisusedtocouple(connect)partsofthepersonalfallarrestsystemandpositioningdevicesystemstogether.Itmaybeanindependentcomponentofthesystem,suchasacaribiner,oritmaybeanintegralcomponentofthesystem(suchasabuckleorD-ringsewnintoafullbodyharness,orasnaphooksplicedorsewntoalanyardorretractablelifeline).
DANGEROUSEQUIPMENT-Equipment(suchaspicklingorgalvanizingtanks,degreasingunits,machinery,electricalequipment,andotherunits)that,asaresultofformorfunction,maybehazardoustoemployeeswhofallontoorintosuchequipment.
DECELERATIONDEVICE-Anymechanism,suchasaropegrab,rip-stitchlanyard,speciallywovenlanyard,tearingordeforminglanyard,automaticselfretractinglifelines,etc.,thatservestodissipateasubstantialamountofenergyduringafallarrest,orotherwiselimittheenergyimposed
onanemployeeduringfallarrest.
DECELERATIONDISTANCE-Theadditionalverticaldistanceafallingemployeetravels,excludingthelifelineelongationandfreefalldistance,beforestopping,fromthepointatwhichthedecelerationdevicebeginstooperate.Itismeasuredasthedistancebetweenthelocationofan
employeesfullbodyharnessattachmentpoint(D-ring)atthemomentofactivation(attheonsetoffallarrestforces)ofthedecelerationdeviceduringafall,andthelocationofthatattachment
pointaftertheemployeecomestoafullstop.
FAILURE-Loadrefusal,breakage,orseparationofcomponentparts.Loadrefusalisthepointwheretheultimatestrengthisexceeded.
FREEFALL-Theactoffallingbeforeapersonalfallprotectionsystembeginsto
applyforcetoarrestthefall.
FREEFALLDISTANCE-Theverticaldisplacementofthefallprotectionattachmentpointontheemployee'sfullbodyharnessbetweenonsetofthefallandjustbeforethesystembeginstoapplyforcetoarrestthefall.Thisdistanceexcludesdecelerationdistance,andlifeline/lanyardelongation,butincludesanydecelerationdeviceslidedistanceorself-retractinglifeline/lanyardextensionbeforetheyoperateandfallarrestforcesoccur.
FULLBODYHARNESS-Strapsthatmaybesecuredabouttheemployeeinamannerthatwilldistributethefallarrestforcesoveratleastthethighs,pelvis,waist,chestandshoulderswithmeansforattachingittoothercomponentsofapersonalfallarrestsystem.
GUARDRAILSYSTEM-Abarriererectedtopreventemployeesfromfallingtolowerlevels.
HOLE-Agaporvoidtwoinches(2")ormoreinitsleastdimensioninafloor,roof,orotherwalking/workingsurface.
LANYARD-Aflexiblelineofwireropeornylonstrapthatgenerallyhasaconnectorateachendforconnectingafullbodyharnesstoadecelerationdevice,lifeline,oranchorage.
LEADINGEDGE-Theedgeofafloor,roof,orformworkforafloororotherwalking/workingsurface(suchasadeck)thatchangeslocationasadditionalfloor,roof,decking,orformworksectionsareplaced,formedorconstructed.Aleadingedgeisconsideredtobean"unprotectedsideandedge"duringperiodswhenitisnotactivelyandcontinuouslyunderconstruction.
LIFELINE-Acomponentconsistingofaflexiblelineforconnectiontoananchorageatoneendtohangvertically(verticallifeline),orforconnectiontoanchoragesatbothendstostretchhorizontally(horizontallifeline),andthatservesasameansforconnectingothercomponentsofapersonalfallprotectionsystemtotheanchorage.
LOWSLOPEROOF-Aroofhavingaslopelessthanorequalto4in12(verticaltohorizontal).
LOWERLEVELS-Thoseareasorsurfacestowhichanemployeecanfall.Suchareasorsurfacesinclude,butarenotlimitedto,groundlevels,floors,platforms,ramps,runways,excavations,pits,tanks,material,water,equipment,structures,orportionsthereof.
MECHANICALEQUIPMENT-Allmotororhumanpropelledwheeledequipmentusedforroofingwork,exceptwheelbarrowsandmopcarts.
OPENING-Agaporvoidthirtyinches(95cm)ormorehighandeighteeninches(55cm)ormorewide,inawallorpartition,throughwhichemployeescanfalltoalowerlevel.
PERSONALFALLPROTECTIONSYSTEM-Asystemusedtoarrestanemployeeinafallfromaworkinglevel.Itconsistsofanchorage,connectors,afullbodyharness,ashockabsorbinglanyardandmayincludeadecelerationdevice,lifeline,orsuitablecombinationsofthese.
POSITIONINGDEVICESYSTEM-Abodybeltorbodyharnesssystemriggedtoallowanemployeetobesupportedonanelevatedverticalsurface,suchasawall,andworkwithbothhandsfreewhileleaning.Thesesystemsmustpreventfallpotentialofgreaterthan1meterandbesupportedwithasecuredpersonalfallprotectionsystem.
PRIMARYFALLPREVENTION-Eliminationoffallexposuresthroughuseofguardrailsystems,aeriallifts,scaffoldsoralternateworkmethodssuchaspreassemblyatgroundlevel.
ROOF-Theexteriorsurfaceonthetopofabuilding.
