程序一：
在签订设备合同前，卖方负责将所销售设备的图纸及技术资料提供给中国特种设备检测研究院进行设计审核，经审核合格，并取得《设计文件鉴定报告》后，方可生产设备。

程序二：
签订合同后，卖方负责向国家质检总局特种设备许可办公室申请首台设备型式试验备案。
PhraseII:
AftertheContractsigned,theSellerisresponsibletoapplythefirstMachineTypeTestRecordfromSpecialEquipmentOfficeofCSEI.
进口机电产品（索道及游乐设施）型式试验备案申请需递交如下资料:
NecessarydocumentsforapplyingTypeTestofimportedMechanicalandElectricalMachine（forRopewayandAttractions）
1、中文版《特种设备制造许可申请书》（编号:
TSZS001-2003）4份）及电子版文件（即磁盘）。
1）“ApplicationformforSpecialEquipmentManufacturing”（code:
TSZS001-2003）4copiesinChineseversionandE-Version（CD-ROM）
2.境外制造工厂签署的“声明”（中文），主要内容包括：
境外制造工厂承诺在中国境内销售的***产品遵守中国现行法规和安全技术规范，并承担因违反中国现行法规、安全技术规范所造成的一切后果。
境外制造工厂承诺为在中国境内使用***产品的用户提供及时周到的支持和服务。
境外制造工厂授权***公司在中国境内办理型式试验备案相关手续，并承担相关质量、安全和法律责任。
2）TheStatementsignedbyOverseasManufacture,withcontentsasfollowing:
“TheOverseasManufacturecommitsthat itsRideproductssoldinChinaare inconformity withChineseregulationsandsafetytechnicalspecifications,andwilltaketheconsequencescausedduetoitsviolationoftheregulationsandspecificationsmentionedabove.
TheOverseasManufacturewillprovideourclientswithgoodandtimelytechnicalsupportandserviceforamusementridessoldinChina,andwilltakealltheconsequencescausedbyitsunqualifiedtechnicalsupportandservice.
TheOverseasManufactureherebyentrust（AuthorizedCompany）tohandletheformalitiesfortypetestrecord-filing,applicationforinstallationoftypetestridesample,andwillundertaketherelatedsafety,qualityand legal consequencesandresponsibilities.”
3.国内代理商营业执照和组织机构代码证的复印件；
3）CopiesofbusinesslicenseandorganizationcodeofdomesticAgent
4.境外制造厂的商业注册文件和相关认证证书的复印件。

5.申请产品的样本（图片）

6）卖方授权给买方代表卖方办理上述程序2，但是卖方将会提供买方所要求的必须资料。
6）ThesellerentitlestheBuyertoproceedwithPHASEIIonbehalfoftheSeller,butSellershallprovidenecessarydocumentstoBuyerattherequestoftheBuyer.
程序三：
卖方持已受理的《特种设备制造许可申请书》约请型式试验机构进行型式试验（填写《游乐设施整机（部件）型式试验申请表》）。
PhraseIII:
Withapproved“SpecialEquipmentsManufactureApplication”,TheSellershallinvitesTypeTestInstitutionfilltheEquipmentwholeride（PartsTypeTestApplicationForm.
程序四：
卖方向江西省质监局特种机电设备处申请安装1台型式试验样机。
PhraseIV:
TheSellerappliestoinstalltheTypeEquipmentfromtheSpecialMechanicandElectricEquipmentOfficeofGuangzhouQualityControlBureau.
申请样机批复需提交资料
NecessarydocumentsforTypeTestApproval
1、《型式试验样机试安装申请报告》1份
1.“TypeEquipmentinstallationApplication”oneoriginfile
2、已受理的《特种设备制造许可申请书》复印件1份
2.Approved“SpecialEquipmentsManufactureApplication”onecopy
3、中国特种设备检测研究院同意约请的《游乐设施整机（部件）型式试验申请
表》
3.“TypeTestApplicationFormfortheWholeAmusementRide（orparts）”approvedbyCSEI
4、制造或销售合同复印件1份

5、申请单位《法人营业执照》复印件1份

8、特种设备安装合同或安装委托书1份（制造单位无安装资格时）

以上所有复印件需加盖申请单位公章，申请时应同时提供原件核对确认。
ApplicantshouldapplyOfficialStampforallthecopiesoftheabovedocuments,meanwhileprovideoriginaldocumentsforverification.
程序五：
凭省局开出的《样机批复函》，卖方委托安装公司到深圳市市场监督管理局办理《安装告知》。
PhraseV：
TheSellerentrustsInstallationcompanytodealwithInstallationApprovalwithreplied“TypeEquipmentInstallationApplication”fromShenzhenQualitySupervisionBureau.
安装告知所需资料：
NecessarydocumentsforInstallationApproval
1、《九江市特种设备安装（改造、维修）告知表》（原件1份）；
1.Anoriginof“ShenzhenSpecialEquipmentsInstallation（Modification/Maintenance）Application”
2、《特种设备安装改造维修许可证》（加盖单位公章的复印件1份，验原件）；
2.Acopyof“SpecialEquipmentInstallation/ModificationLicense”（withcompanystampandorigindocumentsprepared）
3、施工人员特种设备作业人员证（加盖单位公章的复印件1份，验原件）；
3.CopyofConstructionOperators’LicenseofSpecialEquipments（withcompanystampandorigindocumentsprepared）
4、使用单位机构代码证复印件1份；
4.ACopyofUser’sOrganizationcode
5、施工项目合同及日常维修保养合同（加盖单位公章的复印件1份，验原件）；
5.AcopyofConstructionContractanddailyMaintenanceContract（withcompanystampandorigindocumentsprepared）
6、项目施工方案及其安全防护措施原件1份；
6.AnorigindocumentsofConstructionPlanandSafetyMeasurements
7、国产设备：
产品合格证（加盖单位公章的复印件1份，验原件）;
如果特种设备产品合格证上注明的使用单位与告知的实际使用单位不同，应提供由上述双方单位签署的产品委托订货证明或转让证明（加盖双方单位公章的复印件1份，验原件）
如制造厂的设备初次来九江或制造厂的制造许可信息发生变更，窗口受理人员可要求申报单位提供制造厂家加盖单位公章的制造许可证、组织机构代码证和营业执照复印件各1份；
7.ForDomesticEquipmentsneeds:
ACopyofProductQualityCertificate（withcompanystampandandoriginaldocumentsprepared）
IftheUserintheProductQualityCertificateisdifferentfromtheUserintheInstallationApproval,bothUsersshallprovidethecopyofsignedProductOrderEntrustmentorProducttransferringDocuments（withbothcompany’sStampandorigindocumentsprepared）.
IfitisthefirsttimethattheManufacturecometoJIUJIANGorthere’ssomechangeoftheManufactureLicense,theofficerscanrequirethecompanytoprovideacopyofManufactureLicense,OrganizationCodeandBusinessLicensewithcompanystamp.
8、境外生产设备：
大型游乐设施须提供型式试验报告（或省局开出的样机批复函）和《大型游乐设施设计文件鉴定报告》（原件各1份）。
注：
需进行设计文件鉴定的大型游乐设施，见《大型游乐设施设计文件鉴定规则（试行）》（国质检锅[2003]321号））；
8.ForOverseasManufacture:
OrigindocumentsofTypeTestReportofLarge-scaleSpecialEquipments（orInstallationApplicationApprovalbyLocalProvinceBureau）.
Note:
IftheDesignReviewforSpecialEquipmentisneeded,pleaseobeythe“SpecialEquipmentsDesignReviewRules（trialimplement）”（ChineseQualityControlBureauDocumentsNo.[2003]321.
9、如属设备移装的，还需提供下列材料：
a设备在本市内移装的，使用单位需提供移装申请（申请内容应注明其移装设备注册号、原因及移装前后设备安装地点）、设备出厂资料、首次验收检验报告和定期检验报告。
使用单位发生变更的，须先办理过户手续；
b设备从市外移装到本市的，使用单位除提供上述材料外，还应提供设备在原注册地的注销证明。
9.IftheEquipmentsneedtobemoved,followingdocumentsareneeded.
a.Iftherideneedstomovewithinthecity,theUsershallprovidetheTransferringInstallationApplication（including:
RideRegistrationNo,TransferringReasonandNewInstallationplace）,RideManufacturedocuments,InspectionReportoffirsttime,RegularInspectionReport.IftheEquipmentUserchanges,ProductHand-Overprocedureisneedtobedone.
b.Iftheridesmovesintothecityfromothercite,theUsershallprovidealltheabovedocumentsandWrite-offinformationfromoriginregistrationplace.
程序六：
卖方和安装公司在安装自检合格（80小时无故障测试完成）后，买方配合邀请中国特种设备检测研究院检验人员现场进行型式试验和监督检验，并取得《特种设备型式试验报告》。
